Service Visas are given to foreign citizens bearing service passports (or UN passports -- Laissez Passer) on assignment to Indonesia for non-diplomatic purposes. A Service Visa is only Single-Entry.
Based on reciprocity, Service Visas for US citizens are free of charge, provided that the applicant is eligible and completes all requirements. Other nationals must contact us at 1 212 879 0600 for additional information.
Requests for Service Visas must be submitted to an Indonesian diplomatic mission abroad, with the following items: The Application Form (in duplicate) which has been completed and signed by the applicant The original passport with at least six (6) months of validity from the date of submission Two (2) recent color photographs (passport-size) on picture-quality paper A diplomatic note from related US agencies confirming the person's non-diplomatic assignment. For other nationals: a similar diplomatic note from their respective embassy/consulate.
